Civic Address Application

This form is to request a new 911 property number or a replacement sign for your existing 911 property number. These are the blue signs located on your property to identify your address. If you wish to submit an application for a vacant field / farm field, please use the application dedicated to Emily Project Access Signage.

Please note the following, before proceeding with your application:

The installation of a sign and/or post is the responsibility of the property owner. Township staff do not install the sign and/or post.
Payment will be required upon collection of the material at the Township Office by cash or debit.
